Given the extent of adverse events, readmissions, and associated costs to the health care system, there is now increased attention on improving transitions of care. In fact, more and more hospitals are understanding the importance of this issue, and are taking steps related to care transitions that are both improving quality of care and having a positive financial impact. Accordingly, this complimentary program in the Improving Transitions of Care series will explore the current national landscape for transitions of care, and identify where gaps in care are likely to occur.
