We have two options for attending the conference.  You can either attend IN-PERSON in Rosemont, IL or by LIVE WEBCAST.

This program is designed to provide health care professionals with strategies and insights to help organizations be successful in achieving Primary Care Medical Home (PCMH) certification. This program will provide you with the answers, tips, tools, and the guidance you need for your organization to obtain or maintain PCMH certification. Gain insight into the current processes and benefits associated with PCMH certification by Joint Commission, as well as how to sustain performance with these certification standards.

In addition, don't forget to attend the Ambulatory Care Conference on November 8-9, 2023.

Intended Audience: 
Any healthcare related professional interested in Joint Commission’s certification standards for PCMH! You may be a novice, advanced beginner, or an expert wanting to brush up your knowledge and/or skills on Joint Commission’s certification standards for PCMH. This may include, but not limited to, administrators, clinical leadership, managers, and others in the role of registered nurses, physicians, clinicians, etc. As well as several other positions that interface with these certification standards in their role, such as: accreditation/survey managers, quality improvement directors, risk managers.

The attendees will have access to program materials through the Learning Management System until: January 9, 2024.

For conferences held in non-health care settings, The Joint Commission and Joint Commission Resources follow local regulation and CDC community level recommendations as conference requirements related to masking and other interventions to prevent the spread of respiratory disease, including COVID-19.  CDC recommendations are reviewed weekly and any change in requirements will be communicated to conference attendees the Thursday evening before the conference.  We encourage all conference participants to review their organizations travel requirements and assess their own risk and tolerance of possible exposure to respiratory virus infection in a group setting and act accordingly.  This may include wearing the best fitting mask that you can tolerate except when eating and drinking, practicing social distancing when masking is not possible, and choosing to participate in conferences virtually where available. Because the sensitivity of testing for viral pathogens varies based on the test performed, we do not require testing as a condition of participation.  Please do not attend an in-person conference if you are experiencing new onset of respiratory symptoms that could be indicative of viral infection.  However, because we cannot differentiate mild respiratory symptoms due to allergies, resolved infection, and non-infectious causes, attendees with respiratory symptoms should wear a mask.

For your convenience, if you do not have a credit card available, we offer a sales quote/proforma invoice option to assist with initiating your payment. Payments submitted by check take up to 14 days to process. If you are registering for a program less than 14 days away, your registration may not be received in time and your seat may not be reserved. You will not have access to electronic program materials until your payment has been received. We strongly recommend credit card payments for any events less than 14 days away.
